What is your previous experience with MMORPGs? Started with DAoC, played both in EU on Excalibur (Tant Musikant a skald, then webt hib and played Boo the mentalist, then went back to mid and played Audhumla, Dominatricks, Uglok etc, started up the guild New World Order on mid, also had Frelir the friar on prydwen, as well as a runie, vampiir, druid and mentalist on US). Played a warden and assassin in EQ2, played a priest of mithra in AoC, and one month of WoW (after that i couldn't stand WoW)
What are your goals in this game? To enjoy myself and others, and help destruction in the war against order.
Why do you want to join our guild? It's known people from the excal times, so I figured it'd be a nice play to stay
What would you expect of the guild? Fellowship (heh ;)) and people to play and chat with
Have you been in a guild before and if so, why do you prefer us? Founded New World Order on mid/excal and of course played with other guilds over the years, but FoM and TDA are known names from the daoc days to thats why I prefer it.
Do you know anyone from our guild that can vouch for you? I know many by name from excal, but noone I have played a lot with, so I guess not.
What are your normal playtimes? 22-24 CET, some days 20-24 (got family and kid)

I started my very first daoc char a Spirit Master in NWO/Mid but he got deleted after 10-15 levels, some RL friends will join forces with us on release date and they did all start in NWO (that's how I got invited) so you may find some old names you recognise when they join us in Warhammer.
Btw, im a Swede originally aswell :).
